Retake Application

Starting with 2013 exam cycle, the Retake Process is now also fully online. If you cannot login to the Online Application screens and you wish to re-take the exam from any former year, please contact the HQ Office.

Case Logs

Excel version of the ABPh Case Log. Please complete the log(s), save a copy, and upload it with your application or email it to info@theabph.org.

NOTE: If you are documenting 100 ultrasound cases, you DO NOT need to fill out a separate log. You can use the experience case log and fill in the diagnostic ultrasound column as it correlates to each case/patient. If there are multiple cases for a single patient, you can only count the diagnostic ultrasound once; thus, you will need to add additional ultrasound cases to make 100.
