To start your application now, click on Online Application. To submit materials related to your application, click on Upload Documents. The Documentation Checklist is a helpful way to track your progress.
The Reference Materials page holds several files that will be helpful in your developement of related application materials, such as the case log template. Before applying, we strongly urge you to review the eligibility requirements (criteria to sit) posted on the Reference Materials page. All applicants are responsible for knowing and meeting American Board of Phlebology certification examination prerequisites prior to submitting their application.
You may submit your application form anytime during the year. However, to sit for the April 2012 exam your application must be submitted no later than Friday, February 10, 2012. Notification of your eligibility to sit for the exam will be received within 30 days of submitting your application.
Extensions of this deadline are entertained upon request.
Important Dates for the April 2012 Exam
- Candidate Application Period:
October 1, 2011 - February 10, 2012

- Notification to Candidate of Acceptance to Sit for the exam:
November 1, 2011 - March 10, 2012

- Candidate Registration (@ Pearson VUE):
Opens January, 2012 (for approved applicants only)

- Full Refund Cutoff for Exam Cancellation:
March 16, 2012

- Exam Dates:
April 14 - 28, 2012
Note: Pearson Vue centers are close on Sunday, April 22nd.

Candidates are strongly encouraged to register with Pearson VUE as soon as possible after receiving your notification that you may sit for the exam. The earlier one registers, the greater likelihood the requested testing center and date will be available.
