Do I provide two separate logs?

NO. You can submit one log. If your phlebology case has a diagnostic ultrasound related to it, you can indicate it on the same line even if it wasn’t done on the same date. Note: You can only count the diagnostic ultrasound once. So if there are multiple procedures done on a patient, on different days but based on the same diagnostic, you cannot list the ultrasound a second time.
