Case Logs

Excel version of the ABPh Case Log. If you wish to submit the Case Log via email, please send it as an attachment to info@theabph.org.

PDF version of the ABPh Case Log. (Note: if you have Excel, you should use the Excel version.)

NOTE: If you are documenting 100 ultrasound cases, you DO NOT need to fill out a separate log. You can use the experience case log and fill in the diagnostic ultrasound column as it correlates to each case/patient. If there are multiple cases for a single patient, you can only count the diagnostic ultrasound once; thus, you will need to add additional ultrasound cases to make 100.
