Application
Before applying for the ABPh Board Certification, please review the eligibility requirements (criteria to sit). All applicants are responsible for knowing and meeting American Board of Phlebology certification examination prerequisites prior to submitting their application.
The American Board of Phlebology Certification Exam is open to licensed physicians who meet rigorous prerequisite qualifications. The Board does not sanction the term "board eligible." Acceptance for examination is determined after formal application and acknowledges only that the candidate has successfully fulfilled the requirements. It does not certify that he or she is a specialist in phlebology.
Application Requirements
Applicants will be asked to provide the following information and/or
documents:
1. Name
2. SSN
3. DOB
4. Contact info (home and office address, phone, mobile, fax, e-mail).
5. List of all medical licenses held, with license number and date(s) of expiration.
6. A copy of a current, valid, full and unrestricted license to practice medicine in the United States from the jurisdiction in which you practice. Individuals in the military will meet the valid and unrestricted licensing condition if they are on full-time active duty as a physician at a military installation in any branch of the United States Armed Services and submit a valid and unrestricted military medical license. Accompanying this military license must be a signed document from the individual’s commanding officer stating that he/she solely provides medical care and treatment on the military installation to members of the United States Armed Services and their dependents.
7. A copy of an allopathic or osteopathic degree (MD or DO) from an accredited college of medicine, or a letter of verification from the institution where the degree was earned. In the case of foreign medical schools, an English translation of the document is required. Graduates of foreign medical schools who practice in the US must also include a copy of their ECFMG Certificate.
8. Applicants must have completed residency training in a program approved by the Accreditation Council of Graduate Medical Education (ACGME) or the American Osteopathic Association (AOA). A letter from the program director or a notarized copy of certificate of successful completion is required. Alternatively, applicants must now be or ever have been board certified in an ABMS- or AOA-recognized specialty. Proof of such certification is required.
NOTE: The ABPh will temporarily waive the residency requirement described for applications submitted to sit for the exams given in 2008 and 2009.
9. A current Curriculum Vitae.
10. The appropriate reference letters as follows:
a. If applying under the Fellowship, Residency, or Distinguished Scholar Paths, see explanations included in Section II of the Certification Exam Prerequisites & Policies.
b. If applying under the Experience Path, complete a Reference Contact Form, listing three physicians who are diplomates of the ABPh, or diplomates of an ABMS- or AOA-recognized certification board. Please note that no more than one reference may be from a partner or associate, and at least two references must be a physician who is familiar with your practice through either referral or direct observation and who is not your partner or associate.
11. Two passport-sized photographs.
12. Application and Examination Fees
Additional documentation may be required, depending on the applicant’s circumstances and prior experience.
Fees
The fee for board certification is $2595 to be included with the application.
The total fee includes an application fee of $495 and an examination
fee is $2100. The application fee is non-refundable. The exam fee will
be refunded if the applicant is deemed ineligible to sit for the exam.
If the applicant is eligible but cancels on or before April 13, 2008,
the exam fee is refundable. Cancellations after that date, including
failure to appear, will result in a $500 fee, and the balance of the
exam fee will be refunded. Candidates may petition the Board regarding
this fee if there are special circumstances.
Additional documentation may be required, depending on the applicant’s circumstances and prior experience.
Fees may be paid using check (made payable to American Board of Phlebology) or credit card (VISA, MasterCard, or AMEX).
Application Deadline
You may submit an application for the ABPh Board Certification Exam
anytime during the year. However, to sit for the May 2008 exam your
application must be postmarked no later than Friday, February 29, 2008.
Notification of your eligibility to sit for the exam will be received
within four weeks of submitting your application, no later than March
31, 2008.
Candidates are strongly encouraged to register with Pearson VUE as soon as possible after receiving notification that you may sit for the exam. The earlier one registers, the greater the likelihood the requested testing center and date will be available.
